QMHP/Case Manager (Jefferson County, Hardin County, and Golden Triangle location)
As a Rehab Clinician/Case Manager, you will be responsible for providing strengths-based training utilizing an evidence-based curriculum that will help develop and build skills to help the adult or child have a more successful existence. The Rehab Clinician will work with our Case Manager to provide the following services: Skill Training & Development, Medication Training & Support Services, and Crisis Intervention Services. The Rehab Clinician will implement the Service/Treatment Plan focused on the adult or child and family needs and recognize their strengths. The plan will address all the client needs identified by the adult, family, child, and any collateral information.
As the Rehab Clinician, your key responsibilities will involve:
As a Rehab Clinician/Case Manager, you will be responsible for providing strengths-based training utilizing an evidence-based curriculum that will help develop and build skills to help the adult or child have a more successful existence. As the Rehab Clinician, your key responsibilities will involve:
- Responsible for providing skill-building training in the home or out in the community: one-on-one skills sessions at home, at the office, or in the community relating to goals and objectives using evidence-based practices and curriculums
- Provides support and crisis/emergency services for the child/family through face-to-face contact, phone contact, or availability by an on-call system (all time spent on crisis assessment and planning, crisis linkage and follow-up, and stabilization must be documented as crisis hours)
- Maintains all client documentation in the electronic record and writes progress notes adhering to requirements set by the Texas Administrative Code.
- Utilize effective listening skills to establish rapport with clients, family members, and other individuals close to the client; provide emotional support and encouragement; and engage family members in the service process.
- Maintains records by regulatory/agency standards. Enters data in a clear, accurate, and timely manner. Also, promptly submits documentation needed for the tracking of output/outcome measures. May be asked to assist in record review or qualitative review processes.
- Responsible for keeping the family and team members informed on progress toward achieving Service Plan goals
- Assist clients with daily living tasks such as behavior management, counseling services to the client, budget management, independent transportation assistance, time management skills, and other tasks to promote independent living.
- Collaborates with court/probation, tribal/community agencies, families, and other natural support systems to ensure clients’ needs are met. Develops a strong working relationship with community partners. Assist clients in practicing effective communication, problem-solving, and anger management techniques in their daily lives.
- Participate in staff meetings, monthly training, company programs, and professional development training as required.
As a Case Manager, your key responsibilities will involve:
You will be responsible for working cooperatively with the Rehab Clinicians, family members, significant others, and community partners for a successful outcome for the individual. You will conduct clinical screenings, assessments, and service eligibility based on the individual needs. Coordinates, links, and authorizes appropriate services and monitors the individual progress with meeting treatment goals. Delivers interventions such as crisis resolution, admission/discharge planning, and clinical reviews. Provides utilization review activities that address eligibility determinations, level of care assignments, authorizations for continued stays, outlier reviews, inpatient admissions/discharges, and utilization management (UM) administrative issues. Involves extensive review and entry of data and communication with both external providers and team members.
Case Manager Responsibilities:
As the Case Manager, your key responsibilities will involve:
- Will be responsible for assisting the individual in identifying the individual's immediate needs and in determining access to community resources that may address those needs;
- Identify the individual's strengths, service needs, and assistance required to address identified needs.
- Identify the goals and actions required to meet the individual's identified needs;
- Maintain open communication with Licensed Clinical Supervisor, including regular weekly updates, attendance at program meetings, discuss participant successes and challenges.
- Responsible for handling and documenting crises and/or emergencies as needed by the client. The Case Manager will be the primary point of contact for client-related issues.
- Provide a positive role model to clients and staff through leadership, self-discipline, and genuine care for the treatment and recovery of clients. All clients and staff members must be treated with dignity and respect.
- Responsible for functioning as the PRIMARY point of contact for Rehab clinicians as it relates to client-related issues.
- Responsible for conducting an initial meeting with the client to discuss the case manager’s role in service delivery.
- Responsible for conducting outreach and service coordination activities for clients on your caseload.
- Participate in staff meetings, monthly training, company programs, and professional development training as required.
- Maintain a professional and positive attitude, confidentiality, and respect for cultural differences.
- Collaborate with therapists, educational staff, psychiatrists, and any other professionals involved in treating our clients.
- Write, coordinate, and participate in developing Individualized Service Plans and actively involve the youth and their guardian or other support persons to ensure the recovery efforts.
- Document and track the progress of the client in the company tracking database.
- Collaborates with court/probation, tribal/community agencies, families, and other natural support systems to ensure clients’ needs are met. Develops a strong working relationship with community partners.
Initial Requirements:
- Applicant must have a Master's Degree applying for Case Manager.
- All applicants must have at least (1) year of paid experience.
