DESCRIPTION To provide effective advocacy and service coordination for assigned client caseload as leader of the interdisciplinary team, ensuring that clients' physical, social, and emotional needs and goals are identified, addressed and met, as directed by the Individual Futures Plan (IFP), ensuring appropriate service plan implementation, follow-up and monitoring is carried out and documentation completed, in accordance with policies, procedures and standards set by UCP Seguin and all applicable state (DHS) and federal regulations and accreditation agency standards, and in a manner that facilitates cross-team collaboration, cooperation and service integration across work unit, department and organizational boundaries.
QUALIFICATIONS Bachelor's degree in social services, psychology or related field as required by the Ill. Department of Human Services (DHS). Minimum of one year of experience working with individuals with developmental disabilities. Possess or obtain 40 hours of DHS mandated Qualified Intellectual Disabilities Professional (QIDP) classroom training within first 6 months of employment and maintain by completing 12 hours of continuing education units (CEUs), annually. Bilingual (Spanish) is preferred.
Must obtain valid Illinois Driver's License; Vehicle and Proof of auto insurance and maintenance of such at minimum amount of coverage required by law.
