Responsibilities:- Ensures preliminary and comprehensive assessments are developed, updated, and maintained according to facility standards and state and federal regulations for each senior/client.
- Creates comprehensive care plans for seniors/clients by reviewing all assessments and documentation, and ensures that the care plan is carefully adhered to by other Tabitha employees, including direct reports.
- Coordinates overall continuity of senior/client care by assisting, directing, and monitoring nursing staff, in accordance with the care plan, ensuring that Tabitha policies and procedures are followed and that state/federal regulations and standards governing Long-term/Rehab care are conducted.
- Performs any/all duties of those who report to the nurse manager, as needed. Aids with activities of daily living which includes transferring seniors/clients, with or without assistance. Provides therapeutic care as described in the care plan.
- Provides direction and monitors Med Aides who are delivering medications to seniors/clients. Follows through on the comprehensive care plan as developed for each individual senior/client.
- Ensures that all regulatory requirements are met and maintained for assigned seniors/clients. Ensures that all seniors/clients are being evaluated in accordance with CASPER Quality Measures and QAPI plans. Carefully and consistently ensures that department protocol is followed regarding senior/client care documentation. This means completing and submitting timely and accurate documentation and assuring that direct reports do so as well. (Includes medical record, comprehensive care plan, service plan or any other similar record of Tabitha)
- Assumes duties of House Supervisor and scheduled on-call duties as assigned.
- Ensures all Nursing personnel are performing work assignments in accordance with acceptable nursing standards.
- Assists with assuring adequate and effective TEAMember ratios are in place for all shifts.
- Fosters and maintains an effective working relationship with Seniors/clients and their families, with nursing TEAMembers, and between nursing TEAMembers and other TEAMembers of the interdisciplinary team through effective and consistent communication.
- Communicates expectations, holds employees accountable, and manages consistently. Contributes to the success of Tabitha employees by coaching, developing, encouraging, nurturing, mentoring, challenging, and setting an example for direct reports. This will include working with TEAMembers on correcting performance deficiencies, policy violations, or any other area of concern.
- Assists with recruitment by interviewing job applicants, making hiring recommendations and takes steps to control preventable staff turnover related to supervisory responsibilities.
- Willingly takes on additional duties and/or assignments, assisting with projects and responding to requests as assigned or as necessary.
COMMUNICATION: Must be able to speak, read, write, and understand the English language as well as Medical Terminology related to the job. Teaches classes and conducts training for employees.
WORKSITE CONDITIONS: Works indoors in a facility maintained by Tabitha that will typically be in good repair, clean, well ventilated and well lit.
PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job while utilizing standard and specialized equipment. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability and pass a 50 - pound physical exam demonstrating strength and agility prior to employment and from time to time thereafter at Tabitha’s discretion.
AGE REQUIREMENT: At least age 18.
EDUCATION: Successful completion of a Registered Nurse course of study. Bachelor of Science in Nursing preferred.
EXPERIENCE: One year or more of management experience in a healthcare setting preferred. Maintains a body of knowledge pertaining to current nursing theory and practice, infection control, safety, rehabilitative nursing, dementia care, hospice care, management principles, state/federal regulations pertaining to rehab, dementia, hospice, home care and/or any other relevant aspect of nursing and care for older adults. Knowledgeable of and follows all safety and infection control procedures for position. Able to analyze data and use data to develop solutions.
CREDENTIALS: RN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required. CPR Required. All licenses and certifications must be kept up to date at the expense of the employee. Must meet Tabitha’s vaccination requirements.
