Required qualifications:

Bachelor's Degree from an accredited Physical Therapy Program OR

Master's Degree from an accredited Physical Therapy Program (Based on Graduation Year) OR

D.P.T from an accredited Physical Therapy program (Based on Graduation Year)
Preferred qualifications:

Home Health experience
-
1 year experience as a Physical Therapist
