• Location: Chicago, IL (South/Central Chicago)
• Duration: ASAP Ongoing (3 month locum w/ option to extend)
• Schedule: Monday-Friday 8am-5pm 3-4 days per week, flexible
• Scope: Outpatient FQHC Community Health Psychiatric Mental Health services
• Team: Integrative care approach. Psychiatric consultant supports the Primary Care Provider and Behavioral Health Care Manger in treating behavioral problems.
• Volume: 14-16 PPD
• Ages: All
• EMR: Athena
• Prefer: Local, bilingual Spanish
• Support: Multispecialty clinic with full support staff
• Require: National Cert., IL License/DEA, 1-2 years experience in Psych services as NP
• Credentialing: 60 days
