Project Coordinator, Research and Population Health - Caring Health Center Inc

General Description:

The Research and Population Health Project Coordinator is responsible for the administration and coordination of projects and grants focused on virtual care, digital patient engagement, and quality improvement. The Project Coordinator will work under the direction of and in collaboration with the Research and Population Health Project Manager to support the agency's mission to eliminate health disparities through evidence-based, culturally-responsive health promotion programming. In addition to grants administration, the Project Coordinator will facilitate quality improvement efforts to improve the health center's performance on chronic disease measures, and collaborate with interdisciplinary teams to implement and manage educational initiatives focused on virtual care and chronic disease management.

Minimum Requirements:

• Bachelor's degree (or equivalent experience) in Public Health, Education, Management, or related field.
• Oral and written fluency in English (fluency in another language a plus, such as Spanish, Arabic, Russian, Vietnamese, or Nepali).
• Experience in administration and management grants or grant-funded programs, including reporting, budget management, and workplan development.
• Experience or prior involvement in the development, improvement, or evaluation of health programs/services in community settings; quantitative and qualitative analysis skills preferred.
• Strong theoretical understanding of principles of health equity, social determinants of health, community development, and social justice.
• Experience in curriculum/protocol development and group facilitation (e.g., workshops, courses, or focus groups).
• Complete proficiency in Microsoft Office (Word, Excel, and PowerPoint) and Google Workspace (Email, Calendar, Drive, Docs, Sheets, and Slides) applications.
• Strong analytical skills required to identify and address inefficiencies (knowledge of quality improvement and change management principles and methods a plus).
• Experience managing data (experience with Electronic Medical Record systems a plus).
• Ability to develop presentations, deliver presentations, and facilitate meetings with internal teams, stakeholders, and grant funders.
• Experience conducting literature reviews, developing conference posters, and summarizing/presenting scientific data.
• Superior attention to detail and organizational abilities; ability to validate own work as well as team members' work.
• Ability to demonstrate cultural sensitivity with staff, teams, patients, stakeholders, and partners representing diverse cultures, ethnicities, sexual orientations, gender identities, socioeconomic statuses, educational backgrounds, worldviews, literacy levels, and languages.

• Coordinate a variety of evolving grants focused on priority areas, including (but not limited to) virtual care, digital patient engagement, and quality improvement.
• Grants administration responsibilities will include development and monitoring of workplans for multiple grants, budget/fiscal management, data collection and reporting, execution of grant objectives and deliverables, and funder communications/relations.
• Exercise a high level of organization and attention to detail to ensure timely tracking, completion, and submission of all grant deliverables.
• Maintain positive relationships and responsiveness to grant project officers and technical assistance providers, and ensure successful completion of all required meetings and communications.
• Assist in evaluation activities as needed, including development of data collection tools, collection of data, and analysis of quantitative and qualitative data.
• Support quality improvement efforts for clinical measures focused on hypertension, diabetes, asthma, and other wellness areas as required by regulatory bodies and grants. Develop annual quality improvement goals and workplans, implement data collection and tracking workflows, collaborate actively with departmental leadership on resolution of data quality issues, monitor progress toward goals, and present outcomes to cross-departmental Quality Improvement Committee.
• Contribute to the development and production of trainings and training materials in response to community-based priorities and grant objectives. Support the implementation of training for staff, patients, and stakeholders.
• Continuously expand knowledge and understanding of community resources and services, public health prevention practices, and evidence-based intervention programs.
• Develop and strengthen partnerships and bi-directional referral systems with community-based service providers.
• Report to and collaborate actively with departmental leadership; provide thorough communications and updates regarding program operations, successes, and barriers; identify and address inefficiencies and solutions.
• Collaborate with cross-departmental teams (Interpreting/Translation Services, Marketing Department) on development and dissemination of promotional and educational materials and campaigns.

Working Conditions:

• Position requires prolonged periods of sitting at a desk and typing on a computer; ability to stand for prolonged periods of time; ability to perform moderate activities such as climbing stairs, bending, stooping, reaching, and lifting up to 20 pounds at times; and potential exposure to adverse outdoor environmental conditions.
