About the Job
Position Summary
Coordinates an innovative produce distribution program for food insecure patients with diabetes and prediabetes involving community outreach, organization of produce distribution in collaboration with America’s Grow A Row and the Center for Nutrition and Diabetes and provides assistance with educational materials, patient surveys and outcomes for the program.
Primary Position Responsibilities
1. Outreach to CNDM patients who are food insecure.
2. Coordination of produce distribution with AGAR and volunteers.
3. Preparation of educational materials selected for food distribution.
4. Patient survey development and collection.
5. Assistance with data/outcomes collection.
Qualifications
Minimum Education:
Required:
High School Diploma
Preferred:
Bachelors Degree in Health Education, Nutrition, Public Health, Nursing or related degree or current MPH student
Minimum Years of Experience (Amount, Type and Variation):

Experience with community health promotion
License, Registry or Certification:

Valid Driver’s License
Knowledge, Skills and/or Abilities:
