PeaceHealth is seeking a Program Coordinator Advanced Care Planning for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $33.89 – $50.85. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
Job Summary
The Program Coordinator Advance Care Planning (ACP) will be responsible for providing coordination and successful implementation of PeaceHealth advance care planning initiatives throughout their assigned Network. Will provide facilitator training and continued support to PHMG staff, hospital staff, specialty clinic staff and Home and Community staff. Collaborates with community partners on advance care planning.
Details of the position
+ Under the guidance and leadership of the Manager Advance Care Planning, plans, develops and implements ACP infrastructure within network including direct patient care, education, workflow, documentation, measurement and reporting.
+ Coordinates with state agencies to assure PeaceHealth is maintaining compliance and up to date on all changes to laws, documents and shared information regarding ACP and notary guidelines.
+ Coordinates ACP implementation and integration within the network current care continuum model.
+ Engages community organizations within the network in ACP planning, partnering and collaboration.
+ Participates and coordinates network multidisciplinary steering committees to ensure sustainable ACP service across the care continuum.
+ Provides staff or community education programs to promote ACP understanding and competency in conducting ACP services.
+ Reviews ACP referrals and determines resource need.
+ Provides clinic-based ACP services for patients who need one-to-one support.
+ Offers consultations for hospital-based clinicians.
+ Co-manages online resources, including webpage and online learning materials.
+ Achieves ACP planning goals; proactively addresses issues to maintain quality and service.
+ Conducts quality improvement activities to improve ACP Program outcomes/goals.
+ Performs other duties as assigned.
What you bring
+ Bachelor's Degree Required: Healthcare. or equivalent work experience in healthcare industry or
+ Master's Degree Preferred: Social Work.
+ Minimum of 2 years Required: Experience in a health-related field; providing care across a wide variety of care settings including hospice, hospital, clinic, home care or nursing home and
+ Minimum of 1 year Required: Experience in adult education and
+ Required: ACP Instructor/Facilitator training within 12 months of hire.
+ Required Upon Hire: Basic Life Support and
+ Required within 6 months: Notary Public License
