What You Will Do
- Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
- Communicates/collaborates with the work team using a multidisciplinary approach.
- Provides linkage to community resources to facilitate optimal care.
- Provides age specific care to patients/families in all age groups.
- Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
- Maintains a current working knowledge of UH network services and promotes utilization of such services.
- Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.

- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications
Education
- Master's Degree from accredited school of Counseling (Required)
Work Experience
- Hospital/Health Agency experience (Preferred)
Knowledge, Skills, & Abilities
- Attainment of LPCC status is encouraged with one year of eligibility. (Required proficiency)
- Demonstrates ability to interact professionally with physicians, nurses and other members of the healthcare team. (Required proficiency)
- Demonstrates ability to apply theoretical and clinical knowledge in delivery of care. (Required proficiency)
- Multi tasks and prioritizes work. (Required proficiency)

Licenses and Certifications
- Licensed Professional Counselor (LPC) (Required)
- Licensed by the State of Ohio Chemical Dependency Professionals board at the Licensed Chemical Dependency Counselor (LCDC) level is strongly preferred.
Preferred Qualifications and Position Details
Previous work experience with substance use disorders and mothers is strongly preferred. The program is for mothers with substance use histories and their children, and the goal is to provide optimal clinical care for these families. The LPC will: Perform substance use and psychosocial assessments with mothers with substance use disorders and their children, and identifies and connects to appropriate resources; Develop documents and implement a comprehensive plan based on identified psychosocial and mental-behavioral health needs, with particular attention to trauma, the parent-child dyad, and social issues; Facilitate a weekly counseling intervention with pregnant and postpartum mothers with substance use histories; Effectively communicate within the interdisciplinary work team and participate in monthly collaborative meetings; Maintain patient data entry into the REDCAP database for clinical and research purposes; Ensure continuity of care for families as they transition from pregnancy to parenting, and maintain communication between the 3 branches of the RISE (Rainbow Interdisciplinary Substance Exposure) clinics. This is a grant funded position. The schedule is Monday-Friday, day shift. The position is located at UH Rainbow Center for Women & Children.
