PDN-Private Duty Case Manager needed for North Houston! A small company with big dreams. We need a fantastic RN who loves helping people, well versed in hospice care, willing to roll their sleeves and get the work done, submits timely billable notes, and brings a smile to their patients homes. Compensation:$70,000
Responsibilities:- Consult with attorneys and provide recommendations with regard to cost and time-effective ways to accomplish the client’s goals
- Complete legal research to obtain documentation regarding medical records, health insurance, social security, and medical providers
- Collaborate with outside vendors, staff, and attorneys, to manage the law firm’s case load, present case summaries/updates, and ensure that deadlines are met
- Review legal documents to ensure adherence to all legal requirements
- Create legal documents for attorney review
- Completes an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of the nursing process.
- Assesses and evaluates patient's status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
- Responsible for the planning, coordinating, and delivery of quality interdisciplinary home health care services. Compliance with company standards and federal/state guidelines will be maintained at all times.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- High school diploma is required - 2-year degree and Paralegal certification are acceptable, but a Bachelor’s degree is preferred
- Case management process experience is needed - preferably as a personal injury case manager, or related jobs such as legal secretary, paralegal, or legal assistant at a law firm, non-profit, or human services agency
- Legal research and legal document drafting experience is critical
- Computer proficient - specifically with case management software, word processing, and spreadsheet presentation
- Ability to multitask and stay self-motivated
- Valid RN in Texas
- 1 Year RN Experience
- Understand the use of the EMR system
