Responsibilities (which may vary by client):

• Work in collaboration with the RN Case Manager to fulfill the defined patient-specific care plan
• Collaborate with the care team to ensure all patient needs are fully addressed
• Deliver high-quality skilled care to patients
• Thoroughly document care delivery daily in our EMR system
• Contribute to a culture of caring through individual accountability and teamwork

Our Licensed Vocational Nurse approaches each day with a passionate pursuit of caring for patients by working one-on-one with them and helping them to achieve their health goals.
