About the Role:

As a Primary Care Physician, you’ll support patients in a state-of-the-art outpatient care center designed to foster engagement and wellness. You’ll have a manageable patient panel (fewer than 400 patients) which allows for deeper relationships and more meaningful outcomes. This model is designed to help you focus on what you do best—providing personalized, comprehensive care—backed by an experienced multidisciplinary team.

Key Responsibilities:

• Provide high-quality outpatient care to senior patients
• Collaborate with a multidisciplinary team, including medical assistants, nurses, case managers, and community outreach specialists
• Offer comprehensive services including primary care, cardiology, and wellness care
• Utilize cutting-edge tools and resources like ECO, EKG, and lab services available in every center
• Support patients with easy access to care, including fully paid transportation (Uber/Lyft rides)
• Maintain a manageable patient panel with fewer than 400 patients

Ideal Candidate:

• Qualifications:
• Graduate of an accredited MD or DO program
• Current and unrestricted medical license (or willing to obtain in state of practice)
• Board Certification (or eligibility) in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active DEA license and Medicare/Medicaid provider numbers

Experience:

• Background in Family Medicine, Primary Care, or Geriatrics
• A passion for serving the senior population with compassion and expertise

Work Schedule:

• Hours: Monday – Friday, 8:00 AM – 5:00 PM (No evenings or weekends required)

Relocation Support:

2. Follow-up conversation with the Market Medical Director
3. Final interview (in-person or via Zoom)
