Healthcare Network of Southwest Florida (HCN) is seeking a Advanced Registered Nurse Practitioner (APRN) specializing in Geriatrics to join our team. HCN is a Federally Qualified Health Center located in Collier County on the beautiful Gulf Coast of Florida. HCN has been providing primary care to underserved patients for over 40 years.
We provide comprehensive, integrated primary care to more than 50,000 patients at 21 clinic sites throughout the county with more than 75 practitioners. Services include pediatrics, family and internal medicine, an HIV/AIDS treatment program, geriatrics, women's care, dental, and integrated behavioral health. Job Summary:
The Nurse Practitioner, a registered professional nurse with advanced training, functions as a collaborative member of the health care team in the delivery of comprehensive health and health maintenance to patients in primarily, but not exclusively, ambulatory settings. Primary healthcare includes care of geriatric patients with acute episodic or chronic illnesses, emergency care, rehabilitation, preventive health measures, health maintenance and health education services. The Nurse Practitioner may perform medical services when such services are rendered under the supervision of the Medical Director or another staff physician, in the form of being physically present or by discussing the problem via telephone.
Responsibilities and Standards:
- Elicits histories and conducts physicals.
- Maintains ethical and professional standards deemed appropriate by peers.
- Performs other related duties as indicated or when requested by supervisor.
- Educates patient on all aspects of health maintenance, including adults, adolescents and pediatrics.
- Orders appropriate labs, EKGs, x-rays and be able to recognize abnormal findings.
- Prescribes and dispenses medications and treatments as per the practitioners' licensure.
- Initiates requests for consultation or referral with appropriate professionals, clinics, or other health care resources as indicated according to current policy.
- Assesses and triages patients.
- Refers or seeks consultation with physician when a patient has a condition for which the nurse practitioner is not credentialed.
- Participates in QA/QI and Peer Review.

- Efforts are generally responsive to work flow, must be able to work independently and prioritize work on a daily or weekly basis, subject to occasional reprioritization by others

- May be required to perform the duties of other employees, including supervisors/managers, in their absence.

Experience:
- Education/Training Level:
- Graduation from an accredited Nurse Practitioner Certified Program or a Master's Degree in Nursing with Certification as a Nurse Practitioner
- Must maintain required CME credits. Maintains CPR, PALS and other requirements as defined by HCN Credentials Committee.
- Possess a minimum of 3 years experience in Geriatrics
- Licenses and Certifications:
- Must possess a current Florida license to practice as an ARNP.
- Communication Skills:
- Strong verbal and written communication skills
- Bilingual in Spanish and English preferred
- Technology Skills:
- Strong understanding and ability to use Microsoft Office
- Strong understanding of Electronic Health Record preferred
