This program is an innovative, comprehensive care management model serving frail elders aged 55 and above. This is one of 116 PACE programs in America.Our program is expanding to a second site in southern MA in the spring of 2017 and seeks Medical Director level support to manage our current staff of three geriatricians, 1 psychiatrist and three geriatric nurse practitioners as well as MD/NP staff that will join us. As a physician leader in this program, the Medical Director will be responsible for recruiting, supervision and evaluation of these clinicians and will also have a caseload of patients.Key elements of model of care and Medical Director?'s role in program: 11 member Interdisciplinary Team manages the care of our frail elders and supports each other and the participant in achieving care goals. Participants attend our center for Adult Day Health activities as well as receive on-site clinical care including primary care, behavioral health and rehabilitation. Our physicians follow our elders throughout all locations of care including acute, sub-acute, long-term and home care settings. Small Patient panel (approximately 150 patients per Clinical Team). Regular home visits to our participants are encouraged and time is allotted for this effort. This is a completely capitated program which allows us the flexibility and creativity to provide the best, individualized care for our patients. Quality Improvement efforts are a key element of our care model. The Medical Director will lead these initiatives.We are seeking an IM or Family MD with completion of a Geriatric Fellowship program or a BC/BE Geriatrician, 5 plus years of experience in Internal Med or Geriatrics, minimum of 3 years of leadership or administrative experience.Benefits of Employment include:Malpractice Insurance is covered by the Federal Tort Claims Act (FTCA).PAR/Administrative Load: 20 Clinical hours and 20 Administrative hours.Wonderful opportunity to precept residents and students at program.EMR system in place.Continuing Medical Education Stipend of up to $1500 and 5 days of leave per year.Reimbursement of initial and renewal costs of licenses.Five weeks of vacation to start and generous sick and holiday leave.
