We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join an innovative Hospital at Home program, delivering high-quality inpatient-level care in the comfort of patients homes. Conduct comprehensive evaluations and assessments of acutely hospitalized patients. Collaborate with attending physicians and Command Center staff to develop and implement individualized inpatient care plans. Provide exceptional patient care in diverse settings, including hospitals, virtual platforms, and patients' homes. Full-time, day shift Qualifications: Experience: Minimum of 2 years in inpatient advanced practice roles. Adaptability: Comfortable working in a dynamic and evolving environment. Clinical Skills: Strong clinical acumen with the ability to interpret and respond to complex cases effectively. Communication: Excellent verbal and written communication skills, conflict resolution abilities, and empathy to deliver compassionate, patient-centered care.
