***$10,000 Sign-On Bonus**
DUTIES / RESPONSIBILITIES:
- Provide complex primary care and acute illness evaluation and treatment for patients on an as-needed basis via in-person, at-home, or telehealth visits.
- Perform in-person comprehensive assessments on patients and coordinate with the IDT (Interdisciplinary Team) to develop a comprehensive care plan for an assigned patient panel of up to 110 patients.
- Actively participate as a member of the IDT, communicating patient changes, collaborating on care planning decisions, and ensuring patient-centered care in line with the PACE mission.
- Work collaboratively with case management RNs and other IDT members to provide clinical input and care coordination during Transitions of Care scenarios, and participate in facility and family care conferences as needed.
- Conduct post-hospital and post-ED discharge visits within 1 week of discharge, as well as acute visits aimed at reducing ED and hospital utilization for high-risk patients.
- Provide clinical guidance and support to Nurse Practitioners, with the possibility of a supervisory role.
- Serve as a liaison with outpatient and inpatient specialists to enhance care for PACE patients.
- Participate in a rotating telephonic night and weekend schedule every 5-6 weeks.
- Conduct home or SNF visits as needed, based on clinical criteria or standards set by the PACE program.
- Review outcomes, coding, and utilization data, and collaborate with the Medical Director to establish best practices and improve policy and procedures to reduce unnecessary utilization and improve patient health.
- Protect privacy and maintain confidentiality of company procedures and information about employees, patients, and families.
- Follow PACE policies and procedures, participate in required QI activities, staff training, and meetings.
- Maintain professional licensure and certifications.
- Act within the scope of practice.
QUALIFICATIONS (knowledge, skills, abilities):
- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).
- MD or DO degree.
- Board Certification in Internal Medicine or Family Practice.
- Additional certifications in Geriatrics, Hospice and Palliative Care, Addiction Medicine, or Pain are desirable but not required.
- Active California physician license without restrictions.
- Current Basic Life Support (BLS) training.
- State CDS Certification and DEA Registration.
- Current driver's license and proof of auto insurance.
- Current knowledge of chronic care/geriatric issues and best practices.
- Familiarity with behavioral health and substance use management.
- Interest or experience in post-acute care or transitions of care roles.
- Ability to work well in a managed care environment with an interdisciplinary team.
- Strong communication skills, with the ability to build collegial relationships with providers and the care team.
- Creative and enthusiastic commitment to the "triple aim" - dedication to improving the experience, health, and value for PACE patients.
- Interest or experience in community-based healthcare interventions aimed at keeping high-risk seniors in the home setting while providing patient-centered care.
- Excellent clinical, organizational, and communication skills, especially in settings with frail, elderly patients, their families, and the IDT.
- Working knowledge of PACE regulations is a bonus.
