LOCATIONWe have two opportunities at our FQHCs in Hartford, CT and Waterbury, CTMISSION STATEMENTWheeler provides equitable access to innovative care that improves health, recovery and growth at all stages of life POSITION SUMMARY The Lead Family Practice Physician will provide whole person healthcare from infants to older adults in an integrated care setting. The Physician will diagnose and treat acute and chronic illnesses, provide routine health screenings and counseling on lifestyle changes in an effort to prevent illnesses before they develop. The Physician will guide and coordinate all aspects of care as an engaged and active member of a multi-disciplinary treatment team. The Physician will work with the team to achieve the best possible patient outcome in the most cost-effective manner.Other staff include: Medical Director, Psychiatric Nurse Practitioners, Primary Care Nurse Practitioners, Medical Assistants, Patient Care Coordinators, Dietitian Nutritionist, front desk staff and behavioral health staff.This position will serve 20 patients per 8 hour day and will provide supervision to Nurse Practitioner staff.Up to a $10,000 sign-on bonus available! Develops and carries out treatment plans or alternative options as required. Refers patients who need specialized treatments or treatments that cannot be provided in the health center to an appropriate resource or provider and prescribe and administer medication.Provides and maintains optimal management of chronic conditions such as hypertension, asthma/COPD, diabetes, and hyperlipidemia that result in complications and premature death. Provides care that maintains an ongoing engaged, relationship focused on the patients needs over a lifetime.Prevents illness and premature death by providing immunizations, screening services and support for healthy lifestyles.Works with the multi-disciplinary team to create integrated treatment plans that address the patients physical and psychological needs and address the social determinants that impact health outcomes.Works directly with VP of Health Center Operations, Medical Director and clinical staff to gather, review trends and share UDS data; develop pertinent training related to requirements; chart audits for compliance and work with leadership team and clinical champions to encourage compliance and review best practices to improve outcomes and compliance. Maintains an understanding of the facilities and populations served to review trends in diagnosis, demographics, care and treatment to assist with completion of annual risk assessment, as well as providing suggestions for new program development.Compiles data in an organized and easily understandable method for presentation to leadership and staff groups to evaluate trends and report on plans for improvement.Takes lead role with APRNs and provides day to day clinical oversight in the absence of the Medical Director.Assists the department vice president in developing clinical protocols, continued quality improvement and provides recommendations regarding clinical workflow and process improvement.Meets direct service productivity goals of 18-20 visits per day. Demonstrates knowledge of program/department/agency goals and target outcomes and adheres to practice standards and to data collection and reporting protocols.
