Essential Functions & Responsibilities:

• Manage clinical care and rounding at assigned SNFs.
• Prescribe medications and treatments for residents.
• Provide high-quality, patient-centered primary care.
• Timely documentation of patient care in ChartEasy™.
• Available for consultations with facility nursing staff.
• Reliable transportation required.

Shift Structure:

• Rounding time: 7AM-1PM local time.
• Shifts: Daily Monday-Friday, with flexibility.
• Charting can be completed off-site.
