Essential Functions & Responsibilities:
The provider will manage clinical care at assigned skilled nursing facilities (SNFs), including daily rounding, prescribing medications, and providing comprehensive primary care services. Timely documentation in the proprietary EHR system is required, along with availability for consultations with nursing staff. Reliable transportation and punctuality for scheduled shifts are imperative.

Shift Structure:
Rounding occurs from 7AM-1PM local time, with flexibility in start times. Shifts are Monday-Friday, and documentation can be completed off-site.

Compensation and Benefits:
