The primary purpose of the Staff Physical Therapist evaluates and treats patients, communicates with families, physicians and other health team members and maintains documentation of services in the medical records. Conducts in-services and training for facility staff on an ongoing basis.

We are seeking a motivated and skilled PRN Physical Therapist to join our team in providing high-quality care for residents in a long-term care setting. The ideal candidate will evaluate patients, develop treatment plans, and deliver therapy services to improve mobility, function, and quality of life. This role also includes collaboration with the care team, supervision of Physical Therapy Assistants (PTAs), and participation in caregiver education and discharge planning.
We are seeking a dedicated and flexible PRN Physical Therapy Assistant (PTA) to provide high-quality support in a long-term care setting. The ideal candidate will assist in implementing treatment plans developed by a licensed Physical Therapist, collaborate with the care team, and contribute to patient progress while ensuring compliance with all regulations.
Essential Duties and Responsibilities:
- Patient Care and Treatment:
- Deliver treatment plans as directed by the supervising Physical Therapist.
- Assist in evaluating patients and documenting findings under the supervision of a licensed Physical Therapist.
- Provide therapy services to improve mobility, independence, and quality of life for patients.
- Collaboration and Communication:
- Communicate regularly with the supervising Physical Therapist and healthcare team about patient progress, challenges, and treatment updates.
- Support the nursing department by training Restorative Aides in therapy-related care activities.
- Documentation and Compliance:
- Maintain accurate and timely documentation, including daily treatment notes and progress reports, in compliance with state practice acts and payer requirements.
- Ensure that treatment modifications are appropriately recorded according to facility policies.
- Caregiver and Family Support:
- Educate patients' families and caregivers about maintenance programs and therapy techniques for effective post-discharge care.
- Participate in discharge planning to support patients' transition to their next level of care.
- Equipment Management:
- Assist in recommending durable medical equipment (e.g., braces, walkers, canes) to enhance patient mobility and independence.
- Report any issues with therapy equipment to ensure it remains in proper working condition.
- Training and Facility Participation:
- Participate in in-service training programs to enhance knowledge and skills.
- Attend Patient Care Conferences and other relevant meetings as needed.
Qualifications:
Education:
- Graduate of an accredited Physical Therapy Assistant program.
Experience:
- Experience in long-term care or geriatrics preferred.
- Proficient in computer skills, including documentation systems such as Casamba and Point Click Care.
Skills:
- Ability to communicate effectively and work collaboratively with patients, families, and interdisciplinary team members.
- Strong organizational skills and attention to detail.
