Job Title:Physical Therapist Assistant

• Part-time.
• 8:00am - 4:30pm.
• Monday through Friday (flexible) with optional hours available for Saturday.

Location:915 Center Ave. Fort Collins, Colorado (office location).

• This position does require travel and includes compensation for travel.
• We are looking for someone who can support our patients in the Weld County area.

About the Role:

As a Physical Therapist Assistant at Columbine Poudre Home Care,you will:

• Provides direct physical therapy based on directions of physical therapist to relieve pain, restore/develop function, and maintain maximum performance.
• Utilize a variety of physical agents and devices to reduce the need for medications and provide alternatives to surgery.
• Communicate with physicians, family, Physical Therapist, and other parties regarding changes in treatment plan.

What We're Looking For:

Experience:

• Graduate of an accredited Physical Therapist Assistant program.
• Current Colorado Physical Therapist Assistant license.
• 1+ years of experience as a Physical Therapist Assistant; experience with geriatrics and long-term care preferred.

Skills:

• Ability to write routine reports and correspondence with medical staff as well as patient/family members.
• Ability to carry out instructions and provide basic problem solving.
• Must be proficient in database software including Microsoft Word, Excel, and Outlook.

• Collaborative mindset to coordinate care with other skilled clinicians.

Our Total Rewards package includes a salary range of between $31 - $36/hr. depending on experience, with additional travel stipend.
