Advanced Practice Provider APP Nurse Practitioner NP Physician Assistant Overview: The MGH Complicated Pancreatitis Treatment Center (CPTC) is seeking a full-time nurse practitioner or physician assistant to join its outpatient clinic. The CPTC provides comprehensive, multidisciplinary care in an integrated inpatient and outpatient model for patients with sequelae of pancreatitis (pancreatic necrosis, pseudo cysts, chronic pancreatitis, etc.). The MGH CPTC is the only program of its kind in the New England area, and likely the country, and thus is quickly becoming the predominant referral center in the region. The CPTC has recently received attention from two of the largest Pancreatitis specific advocacy groups in the country (National Pancreatitis Foundation and Mission Cure) and are being highlighted on a national scale. The role of the outpatient nurse practitioner or physician assistant (advanced practice provider or “APP”) is to function as the primary touch point for all CPTC patients in the outpatient setting. Under the supervision and direction of the Advanced Practice Provider (APP) Manager and faculty of the General GI team, and in accordance with the rules and regulations of Massachusetts governing nurse practitioner and physician assistant practice, the APP will play an important team role with care delivery and continuity for patients in the CPTC. This includes patients who have transitioned from the inpatient to outpatient settings, outpatient referrals once seen in the MDC, and all urgent visits for this patient population. Additionally, they will take a primary role for preoperative education for patients planned for total (or partial) pancreatectomy with islet auto-transplantation. In addition to clinical activity, the APP in this role will be actively involved in the CPTC program development including hospital outreach, marketing, protocol development and other projects related to the development and growth of the MGH CPTC. This role is a 5 day a week position with a mixture of in person and remote work. Principle Duties and Responsibilities: Clinical: - Will see a variety of in person, virtual/telemedicine, and urgent add-ons seeing outpatients discharged from the Inpatient CPTC service - Includes discharge telemedicine visit within 3-5 days of hospital discharge - In person or virtual appointments every 1-2 weeks for the first 3 months (approx.) depending on individual patient needs and MDC involvement - Includes patients after percutaneous drainage, endoscopic necrosectomy and surgical intervention - Management of tube feeds and titration to resumption of oral intake - Triage and management of outpatient urgent needs - Endocrine and Exocrine insufficiency management in the post-discharge period - preoperative education for patients in the Islet Auto-Transplantation program - Patients initially screened through the MDC who require surveillance and/or ongoing outpatient management - Post-Discharge Acute Pancreatitis Patients - Care Coordination amongst multidisciplinary specialties - Work collaboratively with members of the GI and CPTC team including MDs, other advanced practice providers, nurses, medical assistants, and patient service coordinators to ensure excellent patient outcomes and patient experience. - Evaluate patients, order tests, procedures, and imaging, and follow up on results and document details in the Epic medical record system. Prescribe medications as needed by patient condition. - Deliver patient care instructions and document in Epic Wrap Up section as needed. - Work closely with the team to answer incoming patient questions (Epic In Basket messages), reply to prescription refill requests. - Call patients back as needed to answer/address questions and document in Epic medical record. - Follow-up on information requests from referring providers. - Assist the team with answering/addressing any patient concerns or complaints. - Act as a clinical and administrative resource to other clinical GI sub-specialties. - Required to carry a pager during business hours and collaborate with team members to arrange for coverage during off hours. - Other job duties as requested by Chief/Clinical Chief of Division of Gastroenterology. Educational/Departmental Responsibilities: - Develop and implement routine patient instructions for GI disorders. - Acts as a resource to other care providers with questions about general GI disorders and their management. - Assist with ongoing clinical research via follow-up visits for certain patients being seen in the GI division. - Assist with patient recruitment for clinical research. - Participates as requested in MGH and Mass General Brigham quality initiatives. - Participates in various hospital and system committees as assigned. - Attend internal (e.g GI Grand Rounds) and external education meetings and reads appropriate professional journals to maintain and enhance skills and clinical competence. Is encouraged to advance their medical and technical skills as appropriate to deliver optimum patient care. - Maintain CME requirements for licensure/renewal and board re-certification. Qualifications/requirements: - Master’s degree (nurse practitioner; physician assistant). - 2-3 years of relevant work experience - Board certified by appropriate NP or PA organization. - Motivated individual who has a consistent record of working with diverse healthcare teams. - Great teammate who works in a collaborative manner with all providers and staff. - Willing to adapt to change and be flexible in the setting a very sophisticated patient population. - Possess the maturity, self-confidence, and ability to develop the clinical initiatives to support the management of GI patients. - Maintain excellent patient experience scores. - Maintain requirements for DEA and MCSL licensure/renewal. Skills/ abilities/ competencies required: - Professional knowledge: clinical, healthcare management and marketing knowledge to enable effective interactions and communications with clinicians, healthcare administrators, and marketing professionals. - Time Management: exceptional organizational skills and ability to prioritize effectively and complete tasks; Flexibility to handle multiple tasks and deadline pressures. - Interpersonal/communication: excellent interpersonal, written and verbal communication, and public speaking skills; excellent command of English language, including medical and scientific terminology. - Information Systems/Technology Skills: exceptional computer skills (including operating systems, word processing, electronic mail, Internet, and other office management systems). - EPIC experience is a plus. - Quality Assurance: skills to work efficiently and optimally and aim to do so in all aspects of position; attention to detail. - Critical Thinking/Decision Making: ability to appropriately evaluate all aspects of a situation and to independently make appropriate and timely decisions. - Discretion and Confidentiality: ability to handle sensitive and confidential matters discreetly and to ensure confidentiality guidelines are maintained by staff. - Safety/Compliance: knowledge in aspects of patient safety applicable to position; knowledge of and compliance with institutional, JCAHO, HIPAA, and other regulatory bodies’ standards.
