This is a Full Time position working in the Inpatient department at Northwest Medical Center.
Seeking a candidate who is dedicated to quality patient care across the continuum, working in an acute care setting. Join a collaborative rehabilitation department dedicated to an interdisciplinary and transdisciplinary team-based approach to medicine. In this department, there are opportunities to cross train in other levels of care across Northwest Medical Center locations including acute care, inpatient rehab, skilled nursing, outpatient, and NICU.

May supervise physical therapy assistants, aides, and athletic trainers. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Reports to the Director of Therapy Services.
Job Summary:
Evaluates and assesses the needs of patients and formulates treatment plans. Develops, carries out, and documents treatment programs. Provides education and training for patients, caregivers, and families. Coordinates within the interdisciplinary team including (but not limited to) nursing, physicians, specialists, respiratory therapy, dieticians, case management, ST/OT, radiologists, etc. Considers, plans, and coordinates ongoing treatment plans to facilitate safe and effective discharge.
Requirements:
Will consider a strong new graduate. Excellent mentorship available. Requires a bachelors degree. Master's degree preferred. Certified as a physical therapist in state of practice. Familiar with standard concepts, practices, and procedures within a particular field. BLS required.
Work Schedule:
This is a full time, flexible work schedule. The position is 5 days per week, which includes two weekend days per month. Option for four, 10 hour day shifts to be negotiated.
