Position Summary: Physical Therapist (PT)
As a Physical Therapist at Pinnacle Home Care, you’ll be a key member of our healthcare team, delivering exceptional Physical Therapy services to our patients. Your role involves comprehensive patient assessments, developing and implementing treatment plans, and collaborating with interdisciplinary team members to provide exceptional care.

Position Summary: Physical Therapist (PT)
As a Physical Therapist at Pinnacle Home Care, you’ll be a key member of our healthcare team, delivering exceptional Physical Therapy services to our patients. Your role involves comprehensive patient assessments, developing and implementing treatment plans, and collaborating with interdisciplinary team members to provide exceptional care.
Key Responsibilities:
- Conduct initial and ongoing comprehensive client assessments, including OASIS evaluations.
- Develop and revise treatment plans in consultation with physicians.
- Contribute to the interdisciplinary client care plan.
- Educate and counsel clients and their families about the physical therapy program.
- Supervise Physical Therapist Assistants every 30 days.
- Communicate changes in client status to physicians and obtain necessary orders.
- Collaborate with team members, instructing them on relevant physical therapy practices.
- Document clinical/progress notes promptly according to company policy.
Qualifications:
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current BLS Certification.
- Valid driver's license with a reliable, insured automobile.
- At least one year of experience as a Physical Therapist.
- Home healthcare experience is preferred but not required.
- Experience with electronic medical records is preferred but not required.
- Excellent emotional health and physical stamina to handle standing, stooping, bending, and lifting.
- Self-directed, with strong observational, judgment, and communication skills.
