Physical Therapist, Skilled Nursing Facility. - Medadventures.

We are seeking a skilled Physical Therapist to join a prominent Skilled Nursing Facility in Reading, PA (19611). This position offers a flexible schedule with day shifts from Monday to Friday, and compensation ranges from $45 to $50 per hour, with comprehensive benefits available.

Responsibilities:

• Screen and evaluate residents for Physical Therapy needs.
• Develop individualized treatment programs in consultation with facility staff.
• Communicate evaluation results and treatment objectives to caregivers and families.
• Document all required information within designated time frames.
• Implement and monitor treatment plans, adjusting as necessary.
• Supervise Physical Therapy Assistants and Rehab Aides.

Qualifications:

• Bachelor’s or Master’s Degree in Physical Therapy from an accredited institution.
• Current and valid state license.
• Previous long-term care experience preferred but not required.
