Physical Therapist (PT) - Faith Home Healthcare

We are seeking a PRN Physical Therapist (PT) to join our team in West Des Moines, IA. The ideal candidate will provide Physical Therapy services as per treatment plans, ensuring excellence, compassion, and integrity in care.

• Qualifications: Licensed PT in Iowa, graduate of an approved PT curriculum, 2+ years of clinical experience preferred, home healthcare experience a plus, valid driver's license, and CPR certification.
• Responsibilities: Evaluate patients, establish treatment plans, prepare clinical notes, coordinate care, instruct patients and families, and participate in quality assurance activities.
• Benefits: Family-oriented work environment, mileage reimbursement, scrubs provided, and competitive pay ($70.00 - $110.00 per hour).

Relocation to West Des Moines, IA is required before starting work.
