Home Health Physical Therapist
Care IV Home Health is a locally owned and operated home health agency in Little Rock seeking a full-time Physical Therapist to join our esteemed team. Requirements:
- Active Arkansas Physical Therapy license or registered in the state of Arkansas.
- A minimum of three professional references on the application or upon initial contact.
- Home Health experience is a plus but not required.
Duties may include, but are not necessarily limited to:
- Visit patients within our Little Rock service area to provide care in their homes or assisted living facilities.
- Develop and execute personalized physical therapy care plans for each patient.
- Record patient progress, including admissions, discharges, OASIS assessments, and regular updates to the Director.
- Complete any other assigned tasks.
