Synchrony Rehab, a subsidiary of Trilogy Health Services LLC, is seeking a licensed Physical Therapist at Somerset Court of Cherryville, Heath House, Cleveland House, Shelby Manor, a dynamic and innovative Assisted and Independent Living health campus located in Cherryville, NC, Lincolnton, NC, and Shelby, NC. You must hold an active or pending Physical Therapy (PT) state license to apply for this position.

Qualifications:

Job Summary:
Responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards. Demonstrates accountability for and contributes to program development, quality improvement, problem solving and productivity enhancement in a flexible interdisciplinary environment.

 

Roles and Responsibilities: 
• Provides therapy services including initial assessment, treatment planning and therapeutic intervention in campus locations, home health and/or outpatient care.

• Contributes to interdisciplinary care plans.

• Evaluates patient response to treatment and provides feedback to the rest of the team.

• Documents the course of patient care including progress made and continuing need for treatment.

• Provides patient and family education on tasks and resources that will assist with continued functional independence.

• Functions as a resource for and provides employees with in-service/cross-training on physical therapy knowledge and skill areas.

• Assists with the development of student programs as requested.
• Contributes to the delivery of cost-effective care.
• Reports any complaints and/or grievances made by the residents to the Rehab Program Director or Director of Health Services. Reports incidents or suspected incidents of resident abuse to the Rehab Program Director/Director of Health Services and/or the Executive Director immediately.

• Travel, as needed, to other campus locations and/or to home health assignments in the designated territory.

• Supervises physical therapy assistants in accordance with state practice act requirements.

• Complies with all resident rights per the company’s Resident Rights Policy.

• Maintains professional/technical competencies and proficiencies for job responsibilities.

• Participates in survey preparation activities.

• Other duties as assigned.

 

Qualifications:

Education: Bachelor Degree
Experience: 0-1 years

 

Licenses and Certifications:

Must hold applicable valid and unencumbered state license.
Current CPR Certification for healthcare providers through CPR training that includes hands on practice and in-person skills assessment preferred.

 

Physical Requirements: 
Sitting, standing, bending, reaching, stretching, stooping, walking, and moving intermittently during working hours. Must be able to lift at least 50lbs. Must be able to maintain verbal and written communication with co-workers, supervisors, residents, family members, visitors, vendors, and all business associates outside of the health campus.
