OVERVIEW:
Reports to: Director of Rehab
The Physical Therapist will offer outpatient rehabilitation primarily in older adults' homes. They'll assess, develop, and administer therapy programs to aid recovery, making referrals to other disciplines as needed. They'll tailor therapy to restore, reinforce, and improve performance while educating patients and caregivers on essential skills for independence. Additionally, they'll promote health and offer flexibility in scheduling and work settings.
TASKS:
Screens, examines, and evaluates patients, including history, systems review, and application of appropriate tests and measures. Interprets examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement required to achieve it, identify precautions/contraindications, and design and plan of care.
Develop appropriate treatment goals and methods in collaboration with the patient and caregivers. Services include but are not limited to application of therapeutic techniques in areas such as:
Therapeutic exercise includes aerobic capacity conditioning, balance coordination, agility training, body mechanics and postural stabilization, flexibility exercises, neuromotor development training, relaxation, strength, power, and endurance training
Functional training
Manual therapy
Airway clearance techniques
Integumentary repair and protection
Use of electrotherapeutic, physical agent, and mechanical models
Health and Wellness
Educational & Experience Requirements:
Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility; and
They must have a Master’s degree in Physical Therapy; or
They must have a Master’s degree in a Physical Therapy field in addition to a Bachelor’s degree in Physical Therapy; or
They must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience.
The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent to a degree in Physical Therapy.
SKILLS:
- Must have a valid driver’s license or reliable, independent transportation to and from the patient’s residence.
- Complies with applicable legal requirements, standards, policies, and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act, and HIPAA.
