About the Job
Job Summary:
The Physical Therapist is a member of patient care team within the Department of Home Health and directly accountable to the Patient Clinical Manager regarding Home Care patients. The position requires the individual be clinically competent and knowledgeable of home health regulations Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The therapist assists in evaluating the level of function and helps develop the plan of care and make revisions as applicable. Supervision of LPTA according to regulations. Prepares clinical and progress notes and advises and consults with the patient, family and other members of the health care team to develop individualized, measureable, realistic short term and long term goals for patients. Participates in in-service education and CEU’s as applicable. Performs specific therapy procedures in the home according to the plan of care. Prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Notifies MD of any changes and instructs patient and family in meeting needs. Performs other job duties as assigned.
Performance Expectations:
- Demonstrates ability to operate ultrasound machine selecting appropriate settings, inspecting and troubleshooting any problems with equipment, and perform safe treatment.
- Demonstrate ability to enter all patients charting in EMR documentation meeting state and federal regulations
Qualifications:
Education/Skills
Graduate of an accredited school of physical therapy required.
Work Experience:
One or more years of clinical experience preferred; home health experience preferred.
Certification/Licensure-DUE UPON HIRE
- Permanent License as a Physical Therapist from the Mississippi State Board of Physical Therapy
- Valid Drivers License
and proof of automobile insurance
Additional Certification/Licensure - Obtained based on required timeframe below
- Basic Life Support
