Overview

Join our dynamic Home Health team as a Physical Therapist, where you'll play a pivotal role in providing therapy services to patients in their homes. This position offers the opportunity to impact patients' lives by fostering independence and enhancing their physical and cognitive functioning.

Key Responsibilities

• Conduct Physical Therapy assessments and develop individualized treatment plans.
• Monitor and document patients' responses to treatments and adjust as necessary.
• Educate patients and caregivers on therapy techniques, devices, and home exercise programs.
• Provide supervision to physical therapy assistants and, as appropriate, certified nursing assistants.
• Prepare detailed progress reports and participate in agency development activities.

Qualifications

Required:

• Master’s degree or higher in Physical Therapy.
• Active Physical Therapist (PT) license in the state of Illinois.
• At least 1 year of experience in a healthcare setting.
• Valid driver’s license and reliable transportation for local travel.

Preferred:

• Please provide your Illinois PT license number during the application process.
• Experience in home health care is advantageous but not required; training will be provided for the right candidate.

Schedule and Travel

• Work Hours: Monday-Friday (day shift; start and end times based on patient needs).
• Weekend Rotation: 1 weekend every 6 weeks (Saturday and Sunday day shifts with scheduled patients).
• Holiday Rotation: 2 holidays per year (1 summer, 1 winter on a rotating basis).
• Travel: Local travel required to patient residences.

Compensation and Benefits

• Compensation: PPV-based rates (details attached).
• Sign-On Bonus: $10,000 First-Year Bonus.
