Overview:
Our Award Winning Client is seeking a dedicated Home Health Physical Therapist to join their exceptional care team. As a Home Health Physical Therapist, you will play a crucial role in managing and delivering patient care plans while maintaining the highest standards of service.

Key Responsibilities:

• Provide comprehensive home health physical therapy services to patients in Ligonier and surrounding areas.
• Collaborate with interdisciplinary teams to ensure seamless care delivery.
• Utilize advanced technology to maintain accurate and timely patient records, reducing time spent on administrative tasks.
• Engage in continuous learning and professional development, supported by a network of clinical educators and mentors.
• Self-manage your time effectively, ensuring all critical deadlines for charting are consistently met.
• Demonstrate proficiency with tablets, mobile phones, and Electronic Medical Records (EMR) systems such as Practice Partner EMR.

Qualifications:

• Required:

  • Bachelor’s degree or higher in Physical Therapy from an accredited institution.
  • Valid Physical Therapist license in the state of Pennsylvania (PA).
  • Minimum of one year of experience in healthcare.
  • Experience with Electronic Medical Records (EMR), including but not limited to Practice Partner EMR.
  • Valid Regular Driver’s License (DL).

• Preferred:

  • Please provide your PT license number.

Schedule and Shift Details:

• Monday through Friday, 8:00 AM to 5:00 PM
• One weekend per month
• Two holidays per year

Travel Requirements:

• Local travel to residential homes, with an expectation of spending approximately 20% of your day driving.
