Position Summary:
We are seeking a compassionate, skilled, and dedicated Physical Therapist to join our dynamic healthcare team in our subacute rehabilitation facility. The Physical Therapist will work with patients recovering from surgeries, injuries, or illnesses, helping them regain mobility, strength, and functional independence. In this role, you will develop individualized treatment plans, provide hands-on care, and collaborate with other healthcare professionals to ensure the best outcomes for patients in a supportive, recovery-focused environment.
Key Responsibilities:
Patient Assessment & Evaluation:
Perform comprehensive physical evaluations to assess patients’ mobility, strength, coordination, balance, and range of motion.
Develop individualized treatment plans based on initial assessments, functional limitations, and goals.
Regularly reassess patient progress and adjust treatment plans as needed to optimize recovery.
Therapeutic Interventions:
Administer evidence-based physical therapy treatments including therapeutic exercises, manual therapy, modalities, and functional training.
Provide education and training to patients and their families on techniques to improve mobility, strength, and quality of life.
Utilize assistive devices and equipment to improve patient function and safety.
Collaboration & Communication:
Work closely with a multidisciplinary team, including physicians, nurses, occupational therapists, and speech therapists, to provide comprehensive care.
Document patient progress, treatment plans, and outcomes accurately and in a timely manner.
Participate in interdisciplinary rounds and case discussions, sharing relevant observations and treatment updates.
Patient and Family Education:
Educate patients and families on injury prevention, self-care techniques, and home exercise programs to promote continued progress after discharge.
Provide guidance and counseling regarding rehabilitation goals, potential outcomes, and the recovery process.
Compliance & Quality Assurance:
Ensure compliance with all regulatory and safety standards, including HIPAA and other healthcare guidelines.
Maintain up-to-date knowledge of physical therapy practices, advancements, and treatment modalities.
Contribute to the continuous improvement of clinical practices and patient care protocols.
Qualifications:
Education:
Doctor of Physical Therapy (DPT) degree from an accredited institution.
Licensure/Certification:
Active and valid state licensure as a Physical Therapist.
CPR/BLS certification (or ability to obtain within a specified timeframe).
Experience:
Experience in a subacute rehabilitation, skilled nursing, or inpatient setting preferred.
Knowledge of rehabilitation techniques, patient care strategies, and common conditions encountered in subacute settings (e.g., stroke, joint replacement, post-surgical recovery, neurological disorders).
Strong manual therapy skills and experience with therapeutic modalities.
Skills & Competencies:
Excellent communication and interpersonal skills to work effectively with patients, families, and multidisciplinary teams.
Strong critical thinking and problem-solving abilities.
Ability to work independently and manage a caseload of patients.
Compassionate, patient-centered approach to care.
