Responsibilities for Physical Therapist

• Based on findings from initial evaluation, develop an individualized plan of care to facilitate optimal performance using standardized tests and measures as evidence for improvement.

• Identify need and make referrals to other disciplines to address the comprehensive needs of the patient

• Monitor patients' progress and adjust treatments accordingly.

• Provide supervision for Physical Therapy Assistants, students and support personnel in accordance with state license requirements and professional standards 

• Perform other duties as assigned and require to provide quality care to residents

Qualifications for Physical Therapist

• Experience providing physical therapy to patients in a geriatric long term care facility
• Graduated from an accredited physical therapy program
• Current Physical Therapy license for state of Delaware
• Current CPR certificate
• This position requires good organizational, time management and communication skills.
