We are seeking a casual/PRN Physical Therapist (PT) to perform home health visits. This position is for a PT to provide services to geriatric clients throughout the Twin Cities area. Prior home care experience preferred, but not required. As a home care Physical Therapist, you will be an integral member of a multi-disciplinary that provides rehabilitative care to clients, allowing them to live independently and age in place safely. This position requires a minimum of 3-5 visits per week as well as the ability to take visits withing less than 48 hours if needed (unless a prior availability is already set).

Graduate of an accredited Physical Therapy program
Current and valid PT license in Minnesota
Medicare experience preferred
Ability to work flexible hours, including weekends and on-call rotations
Valid Driver's License and reliable vehicle
Demonstrated ability to work autonomously
