PrimaryPurpose
Assess Senior Care PartnersP.A.C.E. participants' functional mobility and components affecting theirability to live independently in the community. This position is responsiblefor the overall physical therapy program at Senior Care Partners P.A.C.E. Thiswill include providing and recommending therapeutic interventions.
AccountabilityChart Key Roles
Comprehensive assessmentsin the center and care planning.
Provide treatment toimprove/maintain function.
Provide education toparticipants and caregivers.
Oversee wellnessprogramming.
Provide direction andsupport to Physical Therapy Assistants (PTAs).
Duties and Responsibilities
Conducts initial and periodicassessments, identifies, and develops a participant centered plan of care forthe functional needs of the Senior Care Partners P.A.C.E. participant andassures all are completed as described in Senior Care Partners P.A.C.E. policyto support the organizations mission.
Assesses the medical necessity foradaptive and/or durable medical equipment and makes recommendations to theInterdisciplinary Team (IDT).
Initiates order for equipment andperforms quality checks ensuring its meeting the participants needs.
Plans, develops, and implements aparticipant-centered plan of therapy to include therapeutic exercise,modalities, and manual therapy.
Actively participates in participantcentered care planning.
Participates in the transition ofparticipants from the hospital, assisted living, and/or skilled nursingfacilities.
Consults and assists with the Recreational Therapy department inprovision of physical wellness programming groups at the center.
Provides education and training tocaregivers, family members, and friends as needed to support the plan of carefor the participant-centered plan of care.
Functions as a member of the IDT,communicates participant changes to team members and the development of theparticipant centered plan of care.
Supportsand promotes a high level of customer service to our participants, visitors,and internally between departments and staff members.
Must care, support, andcollaborate with everyone involved in participants family using thepatient-family centered care approach.
Initiatesand/or participates in the development of new programming for the benefit of participantsand that correlates with the organizations strategic plan, vision, and values.
Documentsin the participants medical record as required by policy.
Responsiblefor completing all assigned company education and file requirements on time.
Participates in Quality Improvementactivities.
Keeps leader informed of problems orissues.
May provide student oversight.
Performs other duties as assigned.

Qualifications
Doctorate in PhysicalTherapy (DPT) degree required; unless previously grandfathered in under aBachelors or Masters in Physical Therapy.
Must be a graduate of aPhysical Therapy (PT) program approved by the American Physical TherapyAssociation, the Committee on Allied Health Education and Accreditation of theAmerican Medical Association, or the Council on Medical Education of the AmericanMedical Association and the American Physical Therapy Association or otherequivalent organizations approved by CMS.
Must possess a current,unencumbered, active license to practice as a Physical Therapist in the Stateof Michigan.
Minimum of one year of experience working with the frail, elderly,or long-term care population.
Ability to collaborateeffectively with other IDT members.
Detail oriented with goodproblem solving skills.
Strong communicationskills, both written and verbal.
Well-developed ability toshow discretion and maintain confidentiality.
Excellent organizationalskills.
Must be knowledgeable with MS Office Suite,Word, Outlook, PowerPoint & Excel.
Specific Requirements
Possess current CPR certification by AHA or willing and able toobtain as offered by Senior Care Partners P.A.C.E.

Must possess the ability to plan, organize, develop, implement,and interpret the programs, goals, objectives, policies and procedures, etc.,that are necessary for providing quality care.
Immunizationsand vaccines must be current or medical documentation must be provided from amedical provider with reason (s) to request waiver (Hepatitis B, TB, MMR,Varicella, TDaP, Flu, COVID-19).
Must have reliable transportation,current valid Michigan drivers license, and car insurance.
WorkEnvironment
Has a visible presencewhile working in the center and is subject to frequent interruptions.
Communicates with staff, participants,caregivers, the board of Senior Care Partners P.A.C.E. and other organizationsas necessary.
Works beyond normal working hours and onweekends and holidays when necessary.
Maybe subject to exposure to TB, AIDS, and/or Hepatitis B virus.
May work in an unpredictable home environment with variations incleanliness, temperature, behaviors, smoke, animals, and other potentialhazards.
Subject to callback during emergency conditions(e.g., severe weather, evacuation, disaster, etc.).
Required to drive a car in all types of weather.
Fully onsite work environment.
