Summary:
Physical
Therapist day-to-day activities are to maintain their patient caseload of new
and existing patients. To ensure each patient stays on track to complete their
Plan of Care reaching a discharge status.
Works
through any challenges they may be facing related to their treatment and
consults the referring physician when needed. Ensures all documentation is
complete in a timely manner and works closely with the front office and billing
staff to ensure the needs of each patient are met both clinically and
administratively following each insurance expectation and protocols.

Duties and Responsibilities:
- Performs New Patient
Evaluation as prescribed by the referring physician. - Performs Patient
Re-evaluations - Create daily Progress
Notes - Creates Progress Reports
as needed, in accordance with each insurance’s protocols - Performs and creates a
formal Discharge Note & Summary once the patient has reached Maxim
Medical Improvement (MMI) - Reaches out the
referring physician if necessary, regarding the well-being of a patient - Collaborates with
Clinical Manager, Front Desk Staff and Billing Department regarding
patient questions, scheduling, billing questions. - Co-treats with Physical
Therapist Assistant(s)
Qualifications:
- Doctor of Physical
Therapy or Licensed Physical Therapist - Master's Degree or
Doctoral Degree - Computer skills required
- Other skills required:
