Pinnacle Home Care, Florida’s award-winning and largest privately-owned home health agency is currently seeking dedicated and compassionate healthcare professionals to join our team of skilled home heath care providers. The ideal candidate will have a strong commitment to providing quality care to our patients in their homes.
POSITION SUMMARY
Under the direction of a Physical Therapist (PT), the PTA provides client services that have been delegated by the PT.
RESPONSIBILITIES
- Assists in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
- Participates in the preparation of clinical and daily progress notes, all notes are reviewed by the PT and submitted according to company policy.
- As directed, performs routine strengthening and therapeutic exercises and assist the patient with the use of self-help devices.
- Together with the PT, instructs the client and family and assists in teaching exercise programs to the patient and family.
- Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the PT.
- Observes, and records activities and findings in the clinical record and reports to the PT and the physician any adverse reaction to treatment.
- Consults with PT regarding the outcome of home visits and reports physical findings.
- Instructs other health team personnel including, as appropriate, Home Health Aides, as well as family members with respect to certain phases of physical therapy, so that they may work with the client.
- Writes daily progress notes on patients, and submits according to company policy, and prepares periodic summary reports as indicated.
- Participates with staff, clients, and physicians in discharge activities and assists the Physical Therapist in completing the Physical Therapy Discharge Summary.
- Confirms, at least weekly, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits.
- Assumes other duties deemed appropriate and necessary by the PT and as designated by Registered Nurse Case Management and Nursing Supervisor.
QUALIFICATIONS
- Graduate of an accredited Physical Therapy Assistant program or equivalent education.
- Licensed to practice as a PTA in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Minimum of one year's experience in home healthcare preferred.
- Electronic medical record experience is preferred but not essential.
- Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
- Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
- Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
