The Home Health Physical Therapist Assistant will administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.
Principal Responsibilities
Follow plan of care developed by Physical Therapist using a variety of treatment techniques.
Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination
Consult and practice with other health professionals, involved with patients care.
Provide quality, personalized, evidence-based care, and proven interventions, specific to patient’s needs.
Encouraging and help motivate the patient during their recovery process.
Completes required documentation to complete the patients visits and episodes, as required by federal, state and payor guidelines.
Instructs patients and family/significant others in home exercise programs, to facilitate function.
Education/Experience
Successful completion of a Physical Therapy Assistant program approved by CAPTE, Commission on Accreditation in Physical Therapy Education
One year of hospital or home care experience is preferred.
Associate degree in Physical Therapy Assistant
Must have dependable transportation, possess a valid state driver's license and automobile insurance
Licenses/Certificates
Currently licensed by the Illinois State Board of Physical Therapy.
Current AHA CPR certification is required.
Reliable means of transportation, valid IL driver’s license and vehicle liability insurance
Knowledge, Skills and Abilities

Strong organizational and time management
Working Environment
This job operates in a variety of settings, including but not limited to professional office environment, patient’s homes, and car.
Routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions
Possible exposure to blood, bodily fluids and infectious diseases

This position is employed to provide services for White Star, a not-for-profit affiliate of Springfield Clinic.
