This position is PRN, number of shifts may vary throughout the week, day shift: 8am-5pm

What you will do:
The Home Care Physical Therapist Assistant performs care directed by a plan of care formulated by a Licensed Physical Therapist and collaborates care with the patient, caregivers, provider, and in accordance with departmental policies and procedures, regulatory guidelines, best practice to meet the goals of care. The Home Care PTA is a professional therapist who coordinates and directs the home care patient’s care in their home environment (private residence, assisted living, etc.) based on individual needs. The Home Care PTA documents in the patient’s medical record in a timely manner according to agency policy. The Home care PTA attends and actively participates in case conference and team meetings. The Home Care PTA performs on-call responsibilities or rotating to other departments.
Required Education:
Associates Degree as a Physical Therapist Assistant
Preferred Education:
Bachelor’s degree as a Physical Therapy Assistant
Required Skills:
Therapy skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness.

Preferred Skills:
Experience with Home Care Home Base; Experience with smart phones and smart devices
Required Licensure/Registration/Certification:
Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.
Preferred Licensure/Registration/Certification:
Advanced education or certification in area of specialty
Required Experience:
One year experience in a healthcare setting in respective discipline
Preferred Experience:
Experience as a Physical Therapist Assistant in Home Care or Hospice
