Pharmacist Resident - Cottage Health

This 12-month training program is designed to meet the standard guidelines established by the American Society of Health-Systems Pharmacists (ASHP). The Pharmacy Resident is participating in the training program to advance the resident's knowledge, skills, and abilities to become a more competent practitioner. During the various learning experiences, the resident will work closely with the Residency Program Director who will coordinate resident activities, monitor and evaluate performance, and assist in the development, implementation, and completion of resident projects. The resident is also responsible for the efficient and accurate provision of pharmacist care including drug distribution. Upon completion of the program, the resident will develop competencies in areas of acute care, medication management, practice management, and practice related education and training.

MAJOR ACCOUNTABILITIES / SPECIFIC JOB COMPETENCIES

1 Processes medication orders and accurately dispenses medications and IV solutions. Provides services as outlined in approved protocols.

2 Performs pharmacotherapeutic monitoring and provision of pharmaceutical care and research, develops and optimizes pharmacotherapeutic regimens for patients in acute care areas in a routine, continuous manner which utilizes evidence-based medicine and takes into account the age and clinical condition of each patient.

3 Participates in interdisciplinary patient care rounds, medical emergencies, and works collaboratively with medical staff to manage patient specific drug therapy and communicates follow up with the medical team.

4 Participates in research and quality improvement projects which include medication use evaluations, adverse drug reaction and medication error reporting, and clinical intervention documentation. Submits research and scholarly work for publication, platform presentations, and/or poster presentations.

5 Provides formal and informal education to clinical pharmacists, technicians, pharmacy students, postgraduate trainees, allied health professionals, nurses, and physicians on pharmacotherapy related topics.

QUALIFICATIONS

PharmD degree from an accredited college of pharmacy. California Pharmacist license within 120 days of hire date. Basic Microsoft Office
