POSITION SUMMARY
The MTDM pharmacist will participate in a collaborative practice agreement with various providers.
The MTDM pharmacist is responsible for delivering medication therapy management services, including but not limited to, comprehensive medication reviews, disease specific medication reviews, and targeted interventions. This pharmacist will also assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy, and improve clinical outcomes.
The MTDM pharmacist provides a patient-specific medication action plan that includes the assessment of prescribed medications, need for additional medications, patient educational gaps, inappropriate or need for adjustment of doses, adverse drug reactions, and adherence concerns. Clinical recommendations and interventions to healthcare providers is documented and communicated professionally. The MTDM pharmacist assists with managing medication costs through cost-effective interventions, which may include formulary or therapeutic interchanges, combination products, and assessing unnecessary and/or duplicate medication therapies. The MTDM pharmacist has the empathy, patience, and cultural competence to communicate effectively with a diverse member population and various providers and stakeholders.
The MTDM pharmacist performs duties to meet productivity and performance expectations as required or assigned by the Department Director.
PERFORMANCE EXPECTATIONS
The MTDM pharmacy will be able to communicate effectively, work independently under the supervision of the collaborating provider, and have strong clinical background in their area of practice. The pharmacist will maintain an active collaborative practice and immunization license and participate in clinic statistic monitoring such as patient visit volume and clinical patient outcomes. The MTDM pharmacist demonstrates the ability to perform the department specific competencies as listed on the Assessment and Evaluation Tool.
WORK ENVIRONMENT
Includes exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death. Must be able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Some exposure to blood borne pathogens and other potentially infectious material. Is able to move freely about the clinic. Requires the ability to stand and work at a computer 80% of the time. Overall vision is necessary; including reading, depth perception, close-up work and must have a peripheral field of vision. Hearing and speech on the phone as well as in person are necessary. Must be able to communicate verbally in person, on the phone/computer, and in writing.
POSITION SPECIALITIES
Anticoagulation, Heart Failure, Diabetes, Hypertension, Hyperlipidemia, and Weight Management
The MTDM pharmacist will be obtain training in various disease states for patient management. This includes, but is not limited to, anticoagulation, heart failure, and metabolic disorders management. The pharmacist will titrate medications to goal according to established protocols and policies and practice evidence-based medicine for when clinical judgement is required. The pharmacist will learn how to operate various instruments such as point of care machines, blood pressure machines, and order blood work for patient monitoring, per protocol. The pharmacist will work primarily in a hospital-based clinic setting, seeing patients face-to-face and perform tele consults and phone calls for patient management. Some additional tasks that pertain to patient care management include completing prior authorizations, collaborating with other providers and proceduralists for alternative treatment plans, and administering vaccines. The MTDM pharmacist is expected to maintain their clinical knowledge through updated continuing education programs and keeping an active pharmacy, collaborative practice pharmacy, and immunization license.
Upon hire, the MTDM pharmacist may be trained in various clinical areas for patient management as needed.
REPORTING RELATIONSHIP
These positions report to the Department Manager.
