Rotating On Call Weekdays, Weekends and Holidays.

Position Summary:
Coordinate dispensing and monitoring of drug therapies administered in the patient’s home by a nursing agency or by the patient. Provide drug information to allied nursing agencies. Assure the drug therapy is clinically appropriate for the patient’s diagnosis and confirm financial approval for provision of therapy. Establish quality assurance guidelines to insure appropriate response to product or therapeutic discrepancies and provide for ongoing quality improvement. Review patient status and home environment to insure adequate support for administration in the home. Insure inventory amounts sufficient to provide therapy on a continuous basis.

Responsibilities:- Clinical assessment of patient prior to therapy initiation, for appropriateness of therapy and delivery system.
- Monitoring patient therapy to assess efficacy of prescribed therapy.
- Provide support/consultative services to physicians, nurses and other healthcare providers regarding drug therapy decisions. Provide comprehensive inpatient / outpatient drug therapy management by assuming responsibility for monitoring and modifying the drug regimen when consulted by a physician
- Provide in-services for nursing personnel and drug information as needed to various agency clients.
- Preparation and dispensing of sterile products to patient’s homes.
- Utilize drug use evaluation techniques to monitor appropriateness of therapy.
- Responsible for overall reporting/documentation of suspected adverse drug reactions and medication occurrences
- Responsible for ensuring the optimal provision of evidence-based drug therapy for the purpose of achieving outcomes which improve a patient’s quality of life.
- Responsible for counseling patients to improve medication understanding and compliance as applicable within the specialty area.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
- Responsible for rotation of 24 hour call for clinical services. The on-call Clinical Pharmacist must be available at all times after-hours by phone and/or pager to ensure prompt service, if required. The on-call Clinical Pharmacist must show responsible and safe skills when dealing with after-hours consults.
- Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
- Performs other duties as assigned.
Minimum Education:
None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.
Minimum Experience:
None. Home Infusion Experience Preferred.
Licensure Requirement:
Current TN Pharmacist license.
