Pharmacist-Hospital - UnityPoint Health

As a member of an interdisciplinary healthcare team, the Hospital Pharmacist collaborates with providers to optimize medication therapy and dosing, while guiding clinical staff on administration and side effects. Responsibilities include:

• Ensuring accurate medication verification and provider order appropriateness.
• Conducting admission medication reconciliation and providing clinical recommendations.
• Educating patients and staff on medications and participating in continuing education.

Qualifications include a Bachelor of Science in Pharmacy or PharmD, completion of a residency program, and at least one year of hospital experience. Current licensure with the Iowa Board of Pharmacy is required.
