Department: Pharmacy IP
FTE: Full Time, 1.0, 80.00 hours per pay period (2 weeks), Monday - Friday with one weekend per month and a holiday rotation
Shift: Days
Pay: $54.31 - $81.47 / hour. Pay is dependent on applicant's relevant experience
Summary:
Coordinates and provides comprehensive medication management to optimize medication use for addiction-related medical and psychiatric care for adults 18 years and older at the Center for Dependency, Addiction, and Rehabilitation (CeDAR)

-Monitor for regulatory compliance for State Board of Pharmacy, DEA, pertinent REMS programs
-Support CeDAR leadership for compliance with Joint Commission standards
-Provide education to provider staff on new therapeutics
-Support best practices across transitions of care, including medication histories and opioid reversal agent prescribing
-Collaborate with inpatient behavioral health pharmacist
-Precept pharmacy students and residents
-Partner with UCH Pharmacy Operations managers and procurement specialists to optimize medication delivery and storage

Summary:

Requirements:
- Doctor of Pharmacy from an accredited school of Pharmacy.
- State licensure as a Pharmacist and any relevant life support certification as determined at position level.
- Minimum Experience: 5 years specialty experience OR PGY1 or clinical fellowship completion with 3 years' pertinent work experience OR Pertinent PGY2 completion.
Preferred:
- Experience with inpatient behavioral health, addiction medicine, and substance abuse populations is preferred.
Responsibilities:
- Develops and administers drug therapy protocols as a part of an interdisciplinary team.
- Identifies high-risk patients and implements evidence-based interventions to optimize medication-related outcomes.
- Designs and provides patient education and medication management to patients and their families. Mentors pharmacy staff on areas of expertise.
- In conjunction with a patient's healthcare provider, provides, delegates, and coordinates patient care. Develops and leads quality, safety, and affordability-related initiatives at clinic/unit, service line and department levels.
- Documents interventions and their outcomes for use in drug therapy quality improvement efforts. Participates in research and committees for the purpose of evaluating and advancing standards of care.
- The behavioral health/addiction medicine pharmacy specialist coordinates and provides pharmaceutical care for addiction-related medical and psychiatric patients:
