Responsibilities:
- Assessment and Evaluation: Conduct comprehensive assessments of residents' physical abilities, functional limitations, and pain levels to determine appropriate treatment plans.
- Personalized Treatment Plans: Develop individualized care plans tailored to each resident's specific needs and rehabilitation goals.
- Therapeutic Interventions: Implement various interventions, including therapeutic exercises, manual therapy, gait training, and modalities, to promote functional improvement, pain reduction, and overall well-being.
- Collaboration: Work collaboratively with the interdisciplinary team, including physicians, nurses, occupational therapists, and social workers, to ensure coordinated and holistic care.
- Progress Monitoring: Continuously assess and document residents' progress, adjusting treatment plans as needed to optimize outcomes.
- Education and Training: Educate residents and families on home exercise programs, safety precautions, and adaptive techniques to promote independence and prevent further decline.
- Documentation: Maintain accurate and timely documentation of all evaluations, treatments, and progress notes following facility and regulatory standards.
Qualifications:
- Education: Doctor of Physical Therapy (DPT) degree from an accredited program.
- Licensure: Valid state license as a Physical Therapist in Pennsylvania.
- Experience: Experience in a skilled nursing or geriatric rehabilitation setting is preferred, but new graduates are welcome to apply.
- Skills:
- Excellent communication and interpersonal skills to build rapport with residents, families, and staff.
- Strong clinical skills and knowledge of evidence-based practice in geriatric rehabilitation.
- Ability to work independently and as part of a team.
- Compassionate and patient-centered approach to care.
Schedule:
- Full-time: 40 hours per week.
- Monday-Friday, with weekend rotation.
