SUMMARY:
Under the supervision of the Director of Maternal Medicine and the VP of Planning and Community Engagement, the role of the Maternal Health and Centering Coordinator is to supervise the maternal health advocates and support maternal healthcare at all health centers. The Maternal Health and Centering Coordinator oversees the Centering healthcare model and IMPLICIT maternal screenings, and provides staff development training for both. The Maternal Health and Centering Coordinator spends up to half-time working directly with patients, providing health education and care navigation services.
REQUIREMENTS:
- Work with Clinical Director of Maternal Medicine to standardize prenatal care guidelines across Institute sites where possible
- Work with Clinical Director of Maternal Medicine and Maternal Health Champions to resolve care navigation and referral issues as needed
- Collaborate with Clinical Director on new initiatives related to management of prenatal and postpartum patients
- Collaborate with Institute Communications team to Coordinate the development of any new patient education and communications materials regarding prenatal/postpartum care, and revise/update existing education materials annually
- Implement Centering Pregnancy across participating Institute sites
- Oversee professional development of staff, including Doula and CLC training
- Coordinate and co-facilitate annual centering training for new Institute residents and staff
- Provide direct supervision to Perinatal Care Navigators, managing their performance in alignment with the goals and expectations of the perinatal care navigation program
- Conduct team meetings
- Manage Program Capacity and help monitor site prenatal and postpartum patient volume
- Maintain Program Documents
- Complete annual evaluation of Perinatal Care Navigators
- Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time
- Familiarity with maternal and child health and sexual and reproductive health required
- Prior experience with at-risk families, especially pregnant individuals and infants in healthcare and human services setting
- Minimum of 2 years of experience in a healthcare setting
- Experience facilitating group healthcare and/or teaching health education
Qualifications:
- Associates and/or Bachelors
- MPH or Masters Preferred
- 2 or more years of relevant experience
- Certified Lactation Counselor Preferred
- Basic computer and internet navigational skills required
- Computer literacy with Windows-based operating systems and MS Office applications (Word, Excel, Outlook, Powerpoint)
- Familiarity with electronic health records (EHR) technology Preferred
- Bilingual Spanish English Preferred
