OBJECTIVE:
To effectively participate in the Rehabilitation Service Team of the Rouse Home, in the provision of Speech Therapy Services to individual residents and to ensure the coordination of resident care between all rehabilitative disciplines and interdisciplinary care team.
KEY RESPONSIBILITIES:
- Communicates and coordinates ST functions with other disciplines in a constructive manner that helps build team effectiveness and cooperation.
- Communicates with DOR regarding department programs, goals, objectives, problems, and successes.
- Communicates with nursing staff and physicians regarding resident progress and problems as well as department programs.
- Promotes safe working environment and works to eliminate fire and safety hazards including broken or defective equipment and supplies.
- Participates in Care Plan, Department and Medicare Meetings as appropriate.
- Supervises support personnel (Restorative Aide, Rehab Aides) with respect to state laws and professional standards.
- Participates in educational in-services/programs regarding speech therapy to staff, residents, and the community at large.
- Orders supplies and equipment as necessary to maintain optimal resident care standards. Operates within facility guidelines for purchasing of equipment.
- Ensures cleanliness of department and facilities per infection control policies and procedures.
- Adheres to the policies and procedures of the Rouse-Warren County Home. Assists in the development, Review, and implementation of policies and procedures as appropriate.
- Assists in the planning, development, and implementation of new or enhanced programs and services.
- Participates in facility/department QA programs as appropriate.
- May be requested to perform tasks not included in this job description from time to time.
Qualifications
- Valid ST license in the Commonwealth of Pennsylvania.
- Experience in long-term care and outpatient services
- Good interpersonal, oral and written communication skills.
- Good clinical skills.
- Ability to work as part of a self-directed team.
- Knowledge of pertinent State and Federal Regulations and Joint Commission Standards pertaining to the provision of Speech Therapy in long term care/outpatient settings.
Working Conditions
1. Frequent Mobility
- Moving between patient rooms, therapy areas, and administrative spaces.
2. Prolonged Standing or Sitting
- Standing for long periods during therapy sessions or group activities.
- Alternating between sitting during documentation or patient assessments and standing while demonstrating exercises.
3. Handling and Positioning Patients
- Assisting patients with limited mobility into proper positions for therapy, which may involve bending, kneeling, or lifting.
4. Fine Motor Requirements
- Using precise hand movements for tasks like operating therapy devices, manipulating materials (e.g., flashcards or therapy tools), or conducting swallowing evaluations.
5. Exposure to Health Risks
- Need for adherence to strict hygiene protocols to mitigate risks.
