Per Diem RN Hospice for Sierra Vista and the surrounding area
Are you a highly skilled and motivated Registered Nurse with experience in hospice care? Are you passionate about providing compassionate and high-quality care to patients and their families? If so, we have an immediate opportunity for you to join our dynamic and progressive team at Valor Hospice.
Section 1: Duties & Responsibilities
- Complete initial and ongoing assessments of patients and families to determine hospice needs
- Provide professional nursing care and develop care plans based on nursing diagnosis
- Administer medications and treatments as prescribed by physicians
- Assist patients with activities of daily living and provide health care instructions
- Coordinate patient care as a Case Manager
Section 2: Communication
- Maintain accurate clinical notes and communicate changes in patient's condition to physicians
- Coordinate care plan with community health related persons
- Teach self-care techniques and provide health counseling to patients and families
- Ensure a safe environment for patients and provide continuity of care
- Attend interdisciplinary group meetings
Section 3: Additional Duties
- Participate in on-call duties and ensure availability of necessary items and services
- Supervise ancillary personnel and maintain professional knowledge and skills through continuing education
- Contribute to quality assessment performance improvement teams and activities
