About the Job
Riverside University Health System is currently seeking two dedicated Per Diem Clinical Pharmacists to join its esteemed team at the main Medical Center campus in Moreno Valley, CA. Operating 24/7, the Medical Center provides comprehensive care to patients, making a meaningful impact within the community. Both positions offer a day-shift schedule, consisting of 10 hours with rotating weekends and holidays. One position offers full-time hours, the other may provide more flexible scheduling options, up to 40 hours per week.
Clinical Pharmacists play a vital role in actively managing patients in the Medical/Surgical units and various acute care settings, including transitions from the Emergency Department. Collaborating closely with patient care teams, the incumbents will optimize medication therapy and serve as trusted pharmacotherapy experts.
What we are looking for in candidates:
- Pharm D degree preferred
- CA registered pharmacist license required
- Completion of PGY-1/PG-2 residency strongly preferred, or a minimum of 3 years of Clinical Pharmacist experience in an acute care setting providing active clinical care.
EXAMPLES OF ESSENTIAL DUTIES
Examples of daily responsibilities:
Provide comprehensive and individualized pharmacotherapy management, provide direct patient-centered care working alongside physicians, nurses and other hospital personnel.
Identify high-risk patients and implement interventions to improve quality and safety.
Make appropriate evidence-based, patient-centered medication recommendations.
Complete critical patient monitoring and review patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.
Communicate effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assure continuity of pharmaceutical care between shifts and among staff.
Actively participate in antimicrobial stewardship activities.
Manage pharmacokinetics, anticoagulants, erythropoiesis stimulating agents, and parenteral nutrition therapy; IV to PO conversion, and renal dose adjustments.
Review and facilitate non-formulary medication requests.
Ensure survey readiness and perform medication area inspections.
Participate in the management of medical emergencies.
Provide discharge medication review, reconciliation, and counseling as appropriate.
Supervise pharmacy technicians and interns, precepting pharmacy students and residents, providing in-service training to nursing and physician staff.
MINIMUM QUALIFICATIONS
MINIMUM QUALIFICATIONS:
OPTION I
Education: Graduation from a School of Pharmacy accredited by the American Council of Pharmaceutical Education with a Doctor of Pharmacy degree (PharmD).
Experience: One year of pharmacy residency preferred.
OPTION II
Education: Graduation from an accredited college or university with a bachelor's degree in pharmacy.
Experience: Three years of professional-level experience as a pharmacist in a hospital or healthcare facility working in a clinical capacity.
ALL OPTIONS
Knowledge of: Federal, Board of Pharmacy, Joint Commission, and DHS laws, regulations and guidelines pertaining to patient care and dispensing of legend and non-legend pharmaceuticals and poisons; good customer (physicians, nurses and patient) service; professional standards and practices.
Ability to: Provide quality pharmaceutical care; communicate clearly and effectively; work cooperatively with other staff and healthcare providers; organize, and manage routine and new responsibilities; retrieve and process patient and medication information; properly supervise and direct the work of supportive pharmacy personnel.
License/Certificate: Possession of a valid certificate of registration as a Licentiate in Pharmacy issued by the California State Board of Pharmacy and fulfillment of the continuing education requirements.
Preferred: Specialty board certification by the Board of Pharmacy Specialties (BPS) in any specialty and/or Advanced Practice Pharmacist licensure issued by the California State Board of Pharmacy.
Possession of a current and maintained Basic Life Support (BLS) certificate issued by the American Heart Association must be obtained within 30 days of hire.

Pre-Employment

Probationary Period
As an Approved Local Merit System, all County of Riverside employees, except those serving "At Will," are subject to the probationary period provisions as specified in the applicable Memorandum of Understanding, County Resolution, or Salary Ordinance. Temporary and Per Diem employees serve at the pleasure of the agency/department head.
